TITLE: DISTRICT TRANSITION FACILITATOR

STATUS: EXEMPT
CALENDAR: 187

PAY GRADE: AP3
REPORTS TO: SPECIAL EDUCATION DIRECTOR OR SUPERVISOR

QUALIFICATIONS:
• Texas Teacher's Certificate
• Bachelor's Degree or higher
• Special Education Certification (optional)
• Five years’ experience teaching
• Knowledge of Special Education policies and procedures as defined SBOE
• Knowledge of community agencies that serve adults with disabilities
• Knowledge of funding streams of Vocational Rehabilitation, T.D.M.H.M.R. and TEA
• Demonstrated written and oral communication skills
• Skills in program evaluation and the process of Texas Education Agency monitoring system for performance outcomes
• Such alternatives to the above as the Board may find appropriate and acceptable to meet district needs

• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardizations exists.

ESSENTIAL DUTIES AND RESPONSIBILITIES: 
1. Identify community agencies that provide funding, director contracted services and information and referral for students with disabilities
2. Coordinate with Texas Education Agency, Education Service Center, and State Agencies to plan and promote the best practices in Transition.
3. Coordinate with colleges and universities to develop appropriate instructional programs for students with disabilities, and to design partnership agreements with those organizations for training and follow-up.
4. Provide support to principals and teachers in the design and implementation of instructional options for students with disabilities.
5. Assist in coordinating parent meetings for Transition Training.
6. Consult with parents and families for program planning.
7. Ensure that district educational practices meet compliance with state and federal guidelines
8. Participate in meetings, conferences and workshops relating to the curriculum and supervision of the special education program.
9. Recommend staff development activities for district personnel regarding concepts, services and technologies
10. Communicate district curriculum, instructional practices, accomplishments, goals, and instructional initiatives to parents and community; solicit their input on these instructional issues.
11. Observe professional and ethical standards in accordance with generally accepted community standards and the Texas Education Agency Code of Ethics.

5. Keeps principals, special education director and supervisor informed on events and activities of an unusual nature as well as routine matters related to their accountability.

6. Demonstrates flexibility by adjusting rapidly to changes in plans or procedures.

7. Exhibits overall positive professional attitude and cooperation.

8. Consistently supports and cooperates with the administrative and special education
team.

9. Has good attendance and is punctual.

10. Works with appropriate faculty and staff for students’ optimum mental and physical development.

 

11. Performs other tasks and assumes such responsibilities as may be assigned by the Special Ed. Director or Special Ed. Supervisor.
